MERIDIAN INTERNET PRIVACY STATEMENT
We care about your privacy and make reasonable efforts to safeguard information we collect about you. This page describes many of Meridian's privacy policies.
WEBSITE PRIVACY
Our website automatically collects and stores non-personal information from visitors. For example, we can see how many people come to the website, which pages they look at and how long they stay.
This information does not identify anyone personally. We use it to improve the website for our members and visitors.
We will not collect personal information about you through our site unless you provide us with that information voluntarily. "Personal information" includes things like your name, address, Social Security number, email address, or telephone number.
Online Portal Login
Our online portals can provide you with important and useful health information, including health support tools and other services. Because this information is catered to you, we require you to create an account to access the site to help us protect your information and ensure only you have access to appropriate information on the portal.
The first time you use the Member Portal we will ask for your Member ID number, first name and last name, your date of birth, email address, and phone number, and the last 4 digits of your Social Security number. We use this information to verify your identity. We also use it to give you a user name and password for your online account.
Live Chat
If you use the Live Chat service, we use the information you type in to help answer your question. We keep this information for a short time. We use this information for training purposes. Training helps us give you better service.
Throughout our site, there may be opportunities to contact Meridian via electronic mail or "email." Most email is unencrypted. This means that it can be more easily seen by other internet users while it is being sent, without the sender's permission. The sender may not know if someone else sees it.
To protect your privacy, please avoid using email to send Meridian confidential information. Meridian cannot guarantee the confidentiality of any email you send to us through the internet. Use of email is solely at your own risk.

Meridian is regulated under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy and Security Rules. In accordance with these regulations, Meridian provides our members access to our Notice of Privacy Practices which describes member rights and our obligations with regards to individually identifiable health information (often called PHI). The notices below describe how medical information about you may be used and disclosed and how you can get access to this information. Please review the notice for your plan carefully.

Requests for Protected Health Information
You have the right to request, in writing, information we have about you and to get copies of that information. Meridian can deny your request for certain limited reasons. If we deny your request, we will give you a written reason for the denial. We may charge a fee for copying your records. If you would like to request your protected health information (PHI), please contact Member Services either by phone or through Live Chat. Member Services can provide you with a Request for a PHI form and help you with completing the form.
Authorizing Meridian to Share Your Protected Health Information With Specific Individuals
Member Services can assist you in obtaining an authorization to share your information with others by sending you a form that allows us to provide people you choose with access to your protected health information.

Terms and Conditions for Mobile Phone Texting Service
Meridian uses the short code 647526 to send important messages about its members' health care.
By providing your mobile number you are opting in to receive text alerts, members will receive recurring messages from 647526 based on the Medicaid program enrollment file and eligibility.
When you opt-in to the service, we will send you a message confirming your opt-in to 647526 to receive Meridian Alerts. Message and data rates may apply. Message frequency will vary based on your use. Text "HELP" for help. Text "STOP" to cancel.
Do not text any confidential health information to Meridian.
For additional information, text "HELP" to 647526. You may opt-out of the program at any time by texting "STOP" to 647526. Contact us at 888-437-0606 if you have any questions or need assistance.
The following mobile carriers are supported: AT&T, U.S. Cellular, Verizon Wireless, Sprint, Cricket, Boost, and T-Mobile. T-Mobile is not liable for delayed or undelivered messages.

We will only use information you provide if necessary to comply with any state or federal law, regulation, governmental request, or investigation; to avoid liability, or to protect Meridian's rights or property. By submitting information to Meridian through registration forms or otherwise providing information in connection with the Mobile Phone Texting Service, you agree and certify that all information provided is true, accurate, and complete. You agree to not use a false or misleading name or a name you are not authorized to use, if your name is requested. Access to the service may be denied and appropriate legal remedies sought if Meridian, in our sole discretion, determines that any information provided by you is false, incomplete, or inaccurate.
