Encounters Edit 5169 Error – FQHCs

Attention: Federally Qualified Health Centers (FQHCs)

Meridian would like to inform providers of an unidentified system issue related to Edit 5169, which may have resulted in the incorrect denial of certain FQHC encounter claims.

We are pleased to share that the error has been identified and corrected. Any claims with dates of payment on or after April 10, 2025, will no longer be impacted by this issue.

All previously denied claims impacted by Edit 5169 are currently undergoing systematic reprocessing. Providers do not need to take any further action at this time.
