With MeridianComplete (Medicare – Medicaid Plan), taking care of your patient’s health has never been more rewarding. New in 2024, members can earn points for completing their list of healthy activities through My Meridian Rewards.
Supporting Healthy Choices
My Meridian Rewards also offers helpful tools for managing health in your patient’s daily life. Members can access these resources through a computer, tablet, or phone.
- To-Do List: List of personalized activities for members to complete to improve their health while earning points toward rewards.
- Rewards Redemption: Pick gift cards from their favorite stores like Walmart®, CVS, Kohli’s® and more. Gift cards are sent directly to members via email or physical mail.
- Health Education: Extensive library of articles, videos, decision tools, and more to guide your patients on their journey to better health.
- Tracking Progress: Connect devices like an Apple Watch, FitBit®, Garmin®, and other supported wearable devices to track health and activity levels.
- Health Information: View prescription record, select preferred language, set communication preferences, and more.
How Does It Work?
First, members need to sign up for My Meridian Rewards. To do is:
- Visit meridian.healthmine.com and select Register Now.
- Members will need their member ID card, date of birth, and email address to make an account.
- Members can also call My Wellcare Rewards customer service at 1-866-550-1307 (TTY 711) Monday through Friday from 9:00AM to 9:00PM EST.
- Members will earn points to redeem for gift cards from their favorite stores like Walmart®, CVS, Kohl’s® and more.+
