Disease Management
Meridian has special Disease Management programs to help our members with asthma, diabetes, CVD (heart problems) and COPD (lung problems) stay healthy.
Meridian's Disease Management program give you access to support from nurses and healthcare staff as well as educational materials and newsletters to help you learn about your disease.
You can join our Disease Management program in one of three ways:
- If you have asthma, diabetes, CVD or COPD, you may be signed up for the program automatically by Meridian.
- You can sign up for the Meridian Disease Management program yourself by calling 1-888-437-0606. You can stop participating at any time.
- Your doctor can sign you up for the program.

Meridian can help you get care and live better if you have asthma or Chronic Obstructive Pulmonary Disorder (COPD).
Asthma and COPD
You can live better with asthma and COPD by taking steps to protect your health. Asthma affects people in different ways. Someone with asthma has very sensitive airways that are susceptible to different triggers like tobacco smoke, allergens such as dust mites, pollen, pets, strong perfumes and air pollution. COPD is a chronic lung disease that leads to limited air flow and issues breathing. It includes conditions like emphysema and chronic bronchitis.
If you have asthma or COPD, you may be eligible for this program. We will reach out to you to explain the program, confirm your eligibility and get you enrolled. A Primary Health Coach will complete an assessment and develop a personal care plan based on your knowledge of your condition, lifestyle behaviors, and respiratory symptoms.
Your health coach can help:
- You understand your medications and why it is important to take them as prescribed
- Symptom management to help you stay healthy and out of hospitals
- Proper use and maintenance of respiratory equipment
- Supporting tobacco cessation
- Depression screening
- Preventive care like flu and pneumonia vaccinations, preventative screenings
- Education materials to help you learn more about your condition and self-management, and much more!
Throughout the program, your Health Coach will work with you to identify any nonmedical needs you may have, barriers to care plan compliance and will address questions regarding condition management.
You can opt in or out of the program at any time, and you are welcome to come back if you change your mind.

Whether you have type 1 or type 2, living with diabetes is not always easy. Everyday choices like what to eat can affect blood sugar and overall health. Meridian's Diabetes Disease Management program provides you with the tools, support and resources you need to stay healthier longer at home.
We will reach out to you to explain the program, confirm your eligibility and get you enrolled. A Health Coach will fill out an assessment and make a personal care plan for you based on your condition, lifestyle, and readiness to change.
Your health coach can help:
- You understand your medications and why it is important to take them as prescribed
- Learn how to check and monitor A1C levels
- Notice signs of low and high blood glucose levels
- Blood pressure and cholesterol management
- Nutrition coaching for carbohydrate counting and weight management
- Support tobacco cessation
- Individualized goal setting and management to reduce risk factors
- such as high blood pressure & high cholesterol levels, glycemic control, excess weight, tobacco use and physical inactivity
- Preventive care like flu and pneumonia vaccinations, preventative screenings
- Education materials to help you learn more about your condition and self-management, and much more!
Throughout the program, your Health Coach will work with you to point out any nonmedical needs you may have, barriers to care plan compliance and will address questions about condition management. You can opt in or out of the program at any time, and you are welcome to come back if you change your mind.

Move more. Stress Less. Be Heart Smart with Meridian. Our Heart Disease Management program can help you manage heart conditions and prevent long term complications. It helps members with heart failure, coronary vascular disease (CVD), high blood pressure or high cholesterol. We will reach out to you to explain the program, confirm your eligibility and get you enrolled. A Health Coach will complete an assessment and create a personal care plan for you based on your condition, lifestyle, and readiness to change.
Your health coach can help:
- You understand your medications and why it is important to take them as prescribed
- Blood pressure and cholesterol management
- Heart-healthy nutrition and weight management health coaching
- Smoking cessation
- You become more fit and physically active
- Depression screening
- Preventive care like flu and pneumonia vaccinations, preventative screenings
- Education materials to help you learn more about your condition and self-management, and much more!

Throughout the program, a health coach will stay in touch to keep you on track to reach your goal.
